Nine Staff Disciplined Over Savita Halappanavar Death
Nine members of staff at University Hospital Galway have faced disciplinary action over the death of Savita Halappanavar, the Health Service Executive (HSE) has confirmed.
The HSE said that the disciplinary action included written warnings for some staff and pre-procedural informal counselling with training and mentoring for others.
No staff were dismissed or suspended.
A further 21 staff members involved in the treatment of Ms Halappanavar were ruled to have "no case to answer".
Ms Halappanaver died at the hospital on 28 October 2012, five days after being refused a termination.
The HSE added that disciplinary procedures are now complete.

The husband of Indian dentist Savita Halappanavar is calling for a public inquiry into her death. Ms Halappanavar died following a miscarriage at Galway University Hospital on 28th October. Speaking on RTÉ's Today with Pat Kenny, Gerard O'Donnell the family solicitor said: "Any inquiry conducted by the HSE does not meet with his approval.

Staff at the University Maternity Hospital Limerick are to meet to discuss taking industrial action over failures by the Health Service Executive (HSE). The Irish Nurses and Midwives Organisation (INMO) said midwives at the hospital are "deeply frustrated" by failures to address notified clinical governance concerns flagged in January 2015.

Savita Halappanavar Inquest Hears Post-Mortem Details
The inquest into the death of Savita Halappanavar has heard details from her post-mortem, as the last witnesses gave their evidence. The cause of Mrs Halappanavar's death was recorded as septic shock, E coli in her bloodstream and a miscarriage at 17 weeks, pathologist Professor Grace Callagy told the inquest.
